Pre-authorized Debit Authorization Agreement

This agreement authorizes Fido, owned and operated by Rogers Communications Canada Inc. (“Fido”) to debit your account to pay your periodic charges for the provision of Fido goods and/or services, including all services provided by Rogers Communications Canada Inc.

You confirm that all persons whose signatures are required to sign on the account noted above have agreed to this authorization and you will promptly notify Fido if there is any change to my account information.

Some things you need to know about automatic payments:

• Payments are withdrawn each month (unless otherwise stated in your service agreement). We’ll send you a bill before each withdrawal which will let you know about the payment amount and withdrawal date. By signing up for automatic payments, you agree that the notification period for each withdrawal could be less than the standard 10 days.
• To remove automatic payments from your account, please do so at least 48 hours before your scheduled withdrawal date. Visit fido.ca/myaccount.
• Fido may not assign your automatic payments withdrawal agreement to another party without providing at least 10 days prior written notice to you.

You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any debit that is not authorized or that is not consistent with this agreement.
